BE IT REMEMBERED, that on this day to wit:  the Twenty-Eighth of December in the year 
of our Lord, One thousand, nine hundred twelve, and the one hundred and 35 of the 
Independence of the United States of America, before me, W. T. O'Reilly, M. D. Chairman 
of the Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and 
for the Parish of Orleans, and the City of New Orleans, personally appeared
Fred Leitz, an undertaker, a native of this city, residing at No. 2409 Tchoupitoulas 
Street, who hereby declares that Patrick Campbell (white) a native of Louisiana, aged 
38, departed this life on the twenty-seventh (27 December 1912) at No.____ St. Thomas
