That he is a bona fide citizen of the county of Levy, State of Florida. That he has resided in the State of Florida continuously since some time the week of ___________of January, 1856. That he is the identical person who enlisted at Fienandina, Fla. (I think) under the name of J. W. Adams on or about the 2nd day of February, 1862, in Company D, Regiment 10th, of the State of Florida, in the service of the Confederate States Army and who was HONORABLY DISCHARGED at Appomattox Court House, in the State of Virginia, on or about the 10th day of April, 1865 on account of surrender of Gen. R. E. Lee’s Army. I was paroled 10th day of April, 1865 at Appomattox Court House, Va. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States Army in the year 1865, and did not desert the service of the Confederate States Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. Micklus Company, County of Columbia, in the State of Florida and that while in actual service in said company “D”, 10th Fla. Regiment, of the State of Florida, and in line of duty as such soldier at Pettersburg and Richmond, State of Virginia, on or about the 9th day of March, 1865. and was honorably discharged at the expiration of his service on the 9th day of April at Appamattox Court House, State of Virginia, he says he was ruptured on one side before he enlisted in the Army; that while in the Army, this rupture got worse from forced marcher lifting heavy gurd and building and constructing breast-works while at Richmond and Pettersburg; he says that now he is ruptured on the other right sides, and has been so for 20 years, but has been getting worse for the past ten years, until now it is a large as his fist on both sides, and that he is wholly unable to earn a livelihood by his manual labor, and that as a direct result of said injuries, thus received in line of duty during the war, I am now unable to gain a livelihood by manual labor; that I have continuously since January 1st, 1885 been a citizen of Florida, that neither I, nor my wife, nor both combined, own real and personal property in the value of $80 in this or any other State, and have not purposely disposed of our property for the purpose of availing ourselves of the provisions of the pension laws of Florida; that I am not otherwise enabled, or in a position to earn, and have no income from any source sufficient for, a livelihood, and that I never deserted the Confederate service, and that I receive a pension from no other source.
